SAFETY ALERT : COSMO MED and Type approved Fire doors – Quality issue for A60, A0 and B-15 Fire doors

Cosmo Co. Ltd has issued a Notice Letter and a Root Cause Analysis of a quality issue that has been discovered in a number of fire doors rated A-60, A-0 and B-15 and produced between the date of November 2013 and of July 2017 .It was found that the various delivered products by Cosmo Co. Ltd had some insulation partly missing from inside the door and do not comply with their type approval certification (MED modules B and D).

Cosmo Co. declares that the investigation found is likely to have occurred during a change of work process, when a new manufacturing equipment was introduced in November 2013.

Cosmo Co. Ltd has proposed a method in order to check doors already installed to verify whether or not they are affected. Owners and operators of vessels fitted with any Cosmo Co. Ltd fire doors should check with the manufacturer for information on whether they are affected or not.

If Owners and operators are found to have vessels affected by this issue, please contact your local Bureau Veritas Marine & Offshore office for further advice. Bureau Veritas Marine & Offshore will be contacting Flag, owners and operators of vessels under its classification known to be affected by this issue.

The UK Maritime and Coastguard agency (MCA) has issued a bulletin to inform operators that a quality issue has been brought to their attention about fire doors manufactured by Cosmo Co. Ltd. Please refer to the MCA SAFETY BULLETIN No. 10 on 30th August 2017 on COSMO MED and Type Approved Fire doors – Faulty A60, A0 and B15 Fire doors.

The Marshall Islands have also issued on 6th September 2017 a Safety Advisory recommending Owners/Operators to verify if any door is affected, and if so to contact the Flag and the Class Society. This Safety Advisory also recommends Owners/Operators to take mitigating measures pending rectification of defects. Owners and operators of vessels fitted with any Cosmo Co. Ltd fire doors are invited to check first with the manufacturer for information on whether they are affected or not. The Owners and operators of affected vessels should take temporary mitigating measures (as described in the Safety Bulletin alert or various Flags safety advisories) in the form of regular fire watch/fire patrols, added precaution in case of hot work, attention to good housekeeping and to arrange fire hoses to be ready near the affected doors. If you are found to be affected by this issue, please contact your local Bureau Veritas Marine & Offshore (BV) office for further advice.
